The PHQ (Patient Health Questionnaire) was developed to facilitate the diagnosis of the most common mental illnesses. The questionnaire is completed by the patients themselves. It consists of 5 modules, depression, anxiety, somatoform disorder, alcohol and eating, which can also be used on their own.

There are translations for 23 languages, as well as a short English-language tutorial on the PHQ.
