Patient Health Questionnaire (PHQ)
The PHQ (Patient Health Questionnaire) was developed to facilitate the diagnosis of the most common mental illnesses. The questionnaire is completed by the patients themselves. It consists of 5 modules for depression, anxiety, somatoform disorders, alcohol-related disorders and eating disorders. The modules can also be used on their own.
There are translations for 23 languages, as well as a short English-language tutorial on the PHQ.
